Assesses and evaluates the needs of patients with multiple disease processes and co-morbidities and develops and/or administers an appropriate plan of care
Documents all changes in patient’s condition and updates care plans and outcomes
Identifies teaching needs and plans appropriate strategies for individuals and families
Monitors quality and appropriateness of services provided
Establishes and maintains professional working relationship with health care providers and other members of the disciplinary teams in providing patient and family care
Adheres to agency infection control and safety policies
Adheres to the organization’s policy in regards to absenteeism and appearance

Qualifications:
Has a medical degree, MD or DO
Has a current and valid medical license
Excellent written and verbal communication skills
Strong clinical and interpersonal skills
Critical thinking and able to function independently
Proficiency with computers and electronic medical records
Ability to assess individual client needs, provide direct care and patient teaching
